Durable Power of Attorney: Making Your Wishes Known

Through a written document called a Durable Power of Attorney for Health Care, you can determine the direction of your medical care at some future time should you become incapacitated and unable to authorize or approve decisions about your care. Botsford strongly supports this law and the spirit of personal choice and human dignity it fosters. If you have not already completed a Durable Power of Attorney for Health Care, we encourage you to do so.

    3. from the Patient Rights Office (248) 471-8696

Many physician offices also have these forms available for their patients.
