In order to ensure positive identification of patient specimens from the time of collection and throughout the testing phase, it is absolutely necessary that every specimen be labeled with the following information:
It is the policy of the Laboratory to reject specimens that are not correctly identified. If an improperly labeled specimen is received, a reasonable attempt will be made to resolve the identification problem, and a note will be made in the patient’s report.
