A Durable Power of Attorney for Health Care is a legal document that expresses your wishes about health care and appoints someone to speak for you (Patient Advocate) if you become physically or mentally ill or injured and cannot make your own health care decisions. Diane Smolen recommends that everyone prepare a Durable Power of Attorney for Health and Psychiatric Care and give a copy of the signed and witnessed document to all of your health care providers. Ms. Smolen says, “Talking to your loved ones about your wishes is crucial for receiving the care you desire and can protect your family from the stress and responsibility of having to make difficult choices about your health care.”
