Along with the Rx, we will need your office to forward us:
We will contact the patient to set-up an appointment for their intake evaluation and pulmonary function test, if one is required.
What are the Covered Diagnoses for Pulmonary Rehab?
Medicare and other insurers are very specific regarding covered diagnoses for pulmonary rehab. The most common covered diagnoses are:
