Birth Certificates

Your baby’s birth certificate is an important legal document. 

Birth certificates are produced by Beaumont Hospital - Farmington Hills' Health Information Services Department for infants delivered here or brought to Beaumont - Farmington Hills immediately following birth.

Following delivery, a Health Information Services Department member interviews parents to obtain information required to complete the birth certificate. You may give your child any first, middle or last name of your choice. One of the parents is asked to review the typed birth certificate and sign it. Please refer to “Information Regarding Your Baby’s Birth Certificate”.    

If you are unmarried and you and the baby’s father wish to acknowledge paternity, you must both sign an “Affidavit of Parentage”.  The baby’s father must provide picture ID and his social security number.  The affidavit must be signed by both parents in the presence of a notary public after the baby’s delivery.  This may be done at no charge while you are in the hospital. 

Arrangements may also be made to sign the “Affidavit of Parentage” for a short time after you are discharged from the hospital.  However, if you choose to acknowledge paternity once the birth certificate information is downloaded to the Michigan Department of Community Health, the State charges to change the birth certificate.  

Acquiring Your Baby’s Birth Certificate
After your baby’s birth certificate is completed and signed, you will be provided with a birth verification, a souvenir footprint card, and an application to apply for certified copies of your baby’s birth certificate.  You send the completed application to the Farmington Hills City Clerk’s office.  There is a $15.00 fee for the first certified copy and a $5.00 fee for each additional copy.  Click here to download a blank application Request for Certified Copies of Birth Record.  

The Michigan Department of Community Health requires registration of a birth certificate within five days following delivery.  The signed, original birth certificate is forwarded by Beaumont - Farmington Hills' Health Information Services Department to the Farmington Hills City Clerk for registration.
