Acquiring Medical Records
Your medical record is maintained in a confidential manner by the Health Information Services (Medical Records) Department. No one has access to your record except when you allow someone with your written consent. Physicians participating in your care, court orders and third party payors also have access to your record.
Access to Your Medical Record
You have the right to view your medical record at a time suitable for both yourself and a Health Information Services (Medical Records) Department staff member.
Once discharged from the hospital, you may request and obtain a copy of your record, which is provided for $0.25 per page. The medical record must be completed by all physicians who participated in your care before it is released.
Acquiring and Completing an Authorization
You may download, print and complete the Authorization Form. You may request an authorization form in person, by mail or via fax. Our fax number is (248) 471-8508.
Submitting Your Request and Receiving Copies
Your completed authorization may be submitted to our Health Information Services (Medical Records) Department in person, mailed or faxed. When receiving copies of your medical record, picture identification is required. We are located in the East Building, ground floor, entered via the Emergency Department entrance. We are open to the public from 8 am to 4pm, Monday through Friday.
There is a charge of twenty-five cents ($0.25) per page to receive a copy of your medical record for your own personal use.
There is no charge if your record is sent directly to a physician or another health care facility.
