This program is designed for those patients requiring more intensive insulin management to promote better glycemic control.
During the initial assessment, conducted by a certified diabetes educator, the patient is introduced to and instructed in carbohydrate counting and insulin management. Subsequent follow-up appointments are based upon the needs of the individual.
The goal of this program is to assist the patient in the achievement of tighter blood glucose control through proactive management.
