You may choose coverage for yourself as an eligible Botsford employee. (Full-time salaried employees are not eligible to enroll in Voluntary Short-Term Disability coverage.)

Enrolling for Coverage

You can enroll for coverage after you have completed 3 months of service as a full-time benefit eligible employee or as a part-time management employee, or 12 months of service as an eligible part-time non-management benefit eligible employee or as a full-time non-benefit eligible employee.

Employees can apply for coverage during the annual voluntary benefits open enrollment period, typically in August of each year. For 2008, the open enrollment period is August 18 - 27, 2008.. New hires that do not elect coverage during this enrollment period will have an opportunity to enroll the following year during the voluntary benefit annual enrollment period.

With respect to the effective date, employees that enroll August 18 - 27, 2008, will have a November 1, 2008 policy effective date.

This coverage provides monthly income benefits coordinated with the dates of your Botsford core disability coverage.
