You may choose coverage for yourself only, you and your spouse, you and your children, or you, your spouse and your children.

Enrolling for Coverage

You can enroll for coverage after you have completed 3 months of service as an eligible full-time employee, or 12 months of service as an eligible part-time employee.

Employees can apply for coverage during the annual voluntary benefits open enrollment period, typically in August of each year. For 2008, the open enrollment period is August 18 - 27, 2008. New hires that do not elect coverage during this enrollment period will have an opportunity to enroll the following year during the voluntary benefit annual enrollment period.

With respect to the effective date, employees that enroll from August 18 - 27, 2008, will have a November 1, 2008 policy effective date.

Covered Critical Illnesses:

  • Heart attack
  • Stroke
  • Invasive cancer
  • Kidney failure
  • Blindness
  • ALS (Lou Gehrig's Disease)
  • Major organ transplant
  • Paralysis of two or more limbs
  • Occupational HIV
  • Carcinoma in situ*
  • Coronary artery bypass surgery*
  • Note: most skin cancer excluded
  • *25% benefit

Critical Illness Insurance is underwritten by Trustmark Insurance Company, Lake Forest, Illinois. Coverage is available to applicants who meet eligibility and insurability requirements. Refer to policy form CACI-82001 and forms HS-12-0000/R, HIV 806 and SB 806 for your state for exact terms and provisions. First diagnosis means the first time a physician identifies a covered condition from its signs or symptoms. If you have had a diagnosis of a covered condition prior to your Effective Date, you may not be eligible for a benefit.
