Diabetes and You
This is a comprehensive educational program that addresses each aspect of diabetes management. A physician prescription is required to enter the program.
The program includes:
- A one to one appointment with a diabetes educator
- Eight hours of group classes with comprehensive diabetes management guidance
- Ongoing contact with the physician to ensure optimal blood glucose control
- Follow-up at six weeks and six months.
Daytime and evening classes are available.
This program is certified by the Michigan Department of Community Health and has obtained national recognition by the American Diabetes Association and the American Association of Diabetes Educators.
